Intuitive surgical, inc.(isi) received the biopsy needle involved with this complaint and completed the device evaluation.Failure analysis (fa) investigation did not replicate nor confirm the customer reported complaint.Functional testing of the device in an attempt to replicate the report complaint is not possible due to the returned condition of the device. An additional observation not reported by the site that is not related to the customer reported complaint was also identified: the biopsy needle was found to have a broken sheath tip.A piece approximately 1.6 mm x 1.23 mm was not returned with the biopsy needle. The root cause is attributed to mishandling/misuse.Verification of the event details cannot be performed because no system logs were available. No image or video was available for review. This complaint is reportable malfunction event as per the following: it was alleged that when the customer was deploying the tip of the biopsy needle, it was not coming out right.Fa acknowledges that a fragment was missing from a portion of the device that enters the patient (distal end).Based on the information provided, there was no report from the customer that a fragment from the instrument fell into the patient during the procedure. While there was no report of harm or injury to the patient, the reported failure mode could likely cause or contribute to an adverse event if it were to recur.
