Product complaint # (b)(4).Patient identifier: (b)(6).Procode: krd/hcg.This report is being submitted pursuant to the provisions of 21 cfr, part 4.This report may be based on information which has not been investigated or verified prior to the required reporting date. This report does not reflect a conclusion by cerenovus, or its employees that the report constitutes an admission that the product, cerenovus, or its employees caused or contributed to the potential event described in this report. If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.A supplemental report will be submitted if new facts arise which materially alter information submitted in a previous mdr report.Complaint conclusion: as reported via the sterling study, 2mm x 8cm galaxy g3 xsft hel coil (glx120208, 30485189) failed to detach.Per the case report forms (crf), the device deficiency did not lead to a serious adverse device effect (sade) or an unanticipated serious adverse device effect (usade). No action was taken, no intervention had not been made and no circumstances had been less fortunate.Immediate pre-procedure angiography on (b)(6) 2021 revealed a ruptured anterior communicating artery aneurysm with the following dimensions: height 6mm, dome 4mm, maximum aneurysm diameter 6.7mm, neck size 4mm, and dome-to-neck ratio 1.0mm.Balloon-assisted coil embolization was subsequently performed on (b)(6) 2021.A total of four cerenovus (2 galaxy g3, 1 galaxy g3 xsft, & 1 galaxy g3 mini) coils were implanted during the procedure with 34% angiosuite packing density. There was no microcatheter kickback (loss of access to aneurysm) experienced. The parent vessel diameter was 2.3mm.The patient was discharged home with self-care on (b)(6) 2021 with modified rankin scale (mrs) score of 0.The device was discarded; therefore, no further investigation can be performed.A manufacturing record evaluation was performed for the finished device 30485189 number, and no non-conformances related to the malfunction were identified. With the information available and without the product available for analysis, the reported customer complaint could not be confirmed.Based on the manufacturing record evaluation, there is no indication that the event is related to the device manufacturing process.Coil difficulty to detach is a known potential complication associated with the use of embolic coils in endovascular embolization of intracranial aneurysms. There may be clinical and procedural factors, including aneurysm/vessel characteristics, device interaction, and operator technique, that may have contributed to the reported event rather than the design or manufacture of the device. The instructions for use (ifu) contains several precautions related to this issue and includes instructions for troubleshooting the situation should it be encountered during use.As part of the cerenovus quality process, all devices are manufactured, inspected, and released to approved specifications. As part of the post market surveillance program, information from this complaint is trended to identify statistical signals for consideration of further correction action.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.
