Section h10: given the nature of the alleged incident, the device could not be returned for evaluation; therefore, a device analysis could not be performed. The clinical/medical investigation concluded that, no relevant supporting clinical information has been provided to assist with a clinical investigation as this was a post market clinical follow up and it was reported further information will not be provided. It was also noted that the patient achieved radiographic union of the fracture at 4 weeks post primary surgery.Per the complaint the patient¿s staph aureus infection at 5 weeks post op reportedly made it necessary for the plate removal, washout and bridging with an external fixator until fracture union.According to the report, the onset date for the reported symptom and the additional surgery performed was not provided. Therefore, the impact to the patient beyond that which has already been reported cannot be confirmed not concluded.Should any additional clinical information be provided, this case would be re-assessed.Device batch number was not provided, thus, an evaluation of the manufacturing records could not be performed.A review of complaint history of the previous 12 months revealed similar events for the listed device, this failure mode will be monitored for future complaints for any necessary corrective actions.A review of the instructions for use documents for d-rad smart pack¿ system revealed that infections, both deep and superficial, have been reported as adverse reactions with internal fixation devices.A review of the risk management file revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to this product and event.Additionally, since the device batch number was not provided, a review of the sterilization records could not be performed. At this time, we have no reason to suspect that the product failed to meet any specifications at the time of manufacture.Factors that could contribute to the reported event include loss of sterility during procedure, post-operative healing issue, injury, patient condition and/or medical history.Based on this investigation, the need for corrective action is not indicated.Should the device or additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary.Internal complaint reference number: case-(b)(4).
