It was reported by the affiliate in japan that during an unknown surgery on (b)(6) 2022, it was observed that the implant detached from the inserter on the micro quickanchor plus preloaded with 3/0 ethibond suture and v-4 needles when it was opened.According to the report, the surgeon attached the implant to the inserter and tried to insert it to the affected site several times, but the device could not be used.The surgery was completed successfully.There were no adverse patient consequences nor surgical delay reported.
