Event Description
During robotic assisted laparoscopic hysterectomy and sacrocolpopexy, the fenestrated bipolar forceps instrument got caught on the cannula of another instrument.As attempts were made to free the instrument, the fenestrated bipolar broke mid-shaft.All parts of the device were retrieved.
