Event or Problem Description
Information was received from a healthcare provider regarding a patient who was implanted with an implantable neurostimulator (ins) for fecal incontinence. It was reported that when the dilator was inserted over the guide wire it must have sheared on the patient¿s calcified foramen causing the radiopaque marker to come off and remain in patient. When we used a new sheath and inserted the lead the leads seem to not have any responses. They tried new sheath and lead.Cause was not known.
