As the lot number for the device was not provided, a review of the device history records could not be performed.The device has not been returned to the manufacturer for evaluation.However, photos and images were provided for review.The investigation of the reported event is currently underway.
 
Event Description
It was reported that during a vena cava filter placement procedure, the filter was allegedly tilted.It was further reported that the filter legs were allegedly detached, migrated and could be seen in the subsegmental branches of the right pulmonary artery.Reportedly, a snare was used to retrieve the filter.There was no reported patient injury.
 
Manufacturer Narrative
H10: manufacturing review: a manufacturing review was not requested as the lot number reported is unknown.Investigation summary: the physical device was not returned for evaluation.Three medical images and two electronic photos were provided and reviewed.The chest and abdominal x-ray of the image shows the filter which is slightly tilted.The filter like object is also found in the right lung field.Based on the image review, the reported positioning problem and detachment issues are confirmed.The provided two photos shows the filter, which is retrieved from the patient.The filter in the photo has at least one leg missing.Hence, based on the photo review, the reported detachment issue is confirmed.Therefore, the investigation is confirmed for the reported positioning problem and the detachment issue.A definitive root cause for the reported positioning problem and detachment issue could not be determined based upon the provided information.Labeling review: a review of product labeling documentation (e.G., procedural instructions, indications, warnings, precautions, cautions, possible complications, contraindications, nursing guide, and unit label) did not find any product labeling inadequacy.H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
