Event or Problem Description
According to the reporter, on (b)(6)2022, in the pediatric gastroenterology department, a child was admitted to have his peritoneal dialysis catheter repaired again as the junction that had been repaired with glue/stick on (b)(6) 2022 did not hold.Nothing unusual was observed on the device prior to use.The nurse was the one responsible for catheter maintenance (cleaning, dressing changes, etc.) and not the patient. The site never used sepsiderm to clean catheter. The patient had no actual infection. The consequence for the patient was the re-admission to the hospital and the main reason was to have his catheter repaired which resolved the issue. There was no reported final patient outcome.
