Event Description

According to the reporter, the unit had cracked lcd screen, button was stuck, and the battery drained super fast after new batteries were placed in 45 minutes.The user tried several new batteries and still had same failure.As per ifu, the unit takes 4 aa batteries.There was no patient involvement.Medtronics initial evaluation of the incident device found that the speaker wire was getting very hot when first powered on due to defective main board and damaged speaker wire on rear case.

According to the reporter, the unit had cracked liquid crystal display (lcd) screen, button was stuck and the battery drained with a new batteries within 45 minutes.Return button was stuck.The user tried several new batteries and still had same failure.It was reported that the unit was not overheating while troubleshooting with bio lab tech.Unit was dropped on the floor and damaged physically.There was no patient involvement.Medtronic's initial evaluation of the incident device found that the speaker wire was getting very hot when first powered on due to defective main board and damaged speaker wire on rear case.
