It was reported that hcp could not get the lead to insert into the extension fully. This was apparent through an impedance test as well as manufacturer representative (rep) and hcp not being able to meet the insertion line market on the lead itself. No contributing factors were identified.Hcp took the lead and extension out more than five times and tried re-inserting but was unable to get it fully inserted.Hcp then switched extension cables and after a few more tries was able to get the lead fully inserted into the extension using forceps with rubber sheaths to protect the lead.Actions taken to resolve the issue were replacing the extension. The issue has been resolved.Hcp switched out a new extension and was able to fit the lead in the extension. No symptoms were reported.
