Reported event: an event regarding crack/fracture involving a mrh femoral component was reported. The event was confirmed via clinician review of the provided medical records.Method & results: product evaluation and results: material analysis, visual, functional and dimensional inspections could not be performed as the device was not returned.Clinician review: a review of the provided medical information by a clinical consultant indicated: "it is difficult to provide any real information regarding the case with the limited information. There is a hinge style implant in the knee.A single view unlabeled x-ray was provided (the second x-ray was obscured). On the x-ray the femoral stem appears angulated compared to the housing of the femoral implant. The event description indicated a revision was performed.Conclusion/assessment: apparent implant fracture or junctional fracture of the stem implant interface of a tumor style long stemmed hinged knee event confirmation: a fatigue failure of the stem at the housing interface of the femoral implant appears to have occurred.Root cause: a root cause for the implant failure cannot be determined from the materials provided." -product history review: review of the device history records indicate devices were manufactured and accepted into final stock with no relevant reported discrepancies.-complaint history review: there have been no other similar events for the lot referenced. Conclusions: it was reported that the patient was revised due to fracture at the stem/femoral component interface.A review of the provided x-rays by a clinical consultant confirmed the event but was unable to identify root cause. It is unclear from the x-rays which component fractured, if not both.Further information such as return of the devices, additional pre- and post-operative x-rays and the revision operative report as well as patient history and follow-up notes are needed to complete the investigation for determining root cause. No further investigation for this event is possible at this time.If devices and/or additional information become available to indicate further evaluation is warranted, this record will be reopened.
