The device was returned to an olympus repair facility, and physical and functional evaluations of the device were performed.Upon a visual inspection of the device, it was noted that there were unidentified foreign materials in the carbon dioxide (co2) gas inlet of the device, which resulted in no air supply.The reported condition was therefore confirmed.Following the removal of the foreign materials from the inlet, the device operated as expected during functional testing.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Event Description
It was reported by the customer that there was no air supply to the high flow insufflation unit.The reported issue was discovered during the reprocessing of the device.Another device was selected for use instead, and the procedure for which the device was intended is unknown.There were no reports of patient or user harm associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's investigation.Please see updates to h6 and h10.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 6 years since the subject device was manufactured.Based on the results of the investigation, it's likely the loss of air supply occurred because there was foreign material clogged inside the co2 gas inlet.However, a final root cause of the foreign material clogged in the co2 gas inlet was unable to be identified.Olympus will continue to monitor field performance for this device.
