Additional Manufacturer Narrative
H10: manufacturing review: a manufacturing review was not required as this is the only complaint reported to date for this product and lot.Investigation summary: one ultraverse 035 pta dilatation catheter loaded in an unknown sheath was received for evaluation.During visual evaluation, stretching was noted to the catheter shaft. No anomalies to the luers/y-body.The distal tip of the sheath was noted to be buckled.During functional evaluation, an attempt was made to remove the balloon from the sheath.Strong resistance was felt leaving the attempt unsuccessful. The sheath shaft detached from its hub.No further testing could be performed due to the condition of the sample.Three photos were provided and reviewed. The first photo showed the ultraverse 035 balloon partially inserted in the introducer sheath. The catheter was seen stretched along a portion of its length. The second and third photos showed the distal end of the sheath which was seen buckled. Therefore, the investigation is confirmed for the reported sheath removal difficulty as the catheter was received loaded in an unknown sheath and attempts to remove it were unsuccessful. However, the investigation is inconclusive for the reported sheath insertion difficulty as functional testing for the same could not be performed due to the condition of the sample. The investigation is confirmed for the identified catheter stretching as stretching was seen on the catheter shaft from both the provided photos and the returned device.A definitive root cause for the reported sheath insertion difficulty, sheath removal difficulty and identified catheter stretching could not be determined based upon the provided information.Labeling review: a review of product labeling documentation (e.G., procedural instructions, indications, warnings, precautions, cautions, possible complications, contraindications, nursing guide, and unit label) did not find any product labeling inadequacy.H10: d4 (expiry date: 09/2024), g3, h6 (device, method).H11: b5, h6 (result, conclusion).H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
