It was reported that cautery at the beginning of a generator change procedure resulted in an out of range right ventricular (rv) lead impedance measurement and a rv lead polarity switch to unipolar. After the cardiac resynchronization therapy pacemaker (crt-p) being replaced was brought out of the pocket and the lv lead was disconnected, asystole occurred because the rv lead was not capturing. It also noted that intermittent pacing spikes showed up on the electrocardiogram (ecg) strip because the crt-p can was intermittently touching the pocket. The crt-p was successfully replaced and the rv lead remains in use.No further patient complications have been reported as a result of this event.
