A visual inspection was performed on the returned device. The reported deformation due to compressive stress was unable to be confirmed; however, polymer damage noted near the tip may have been inadvertently reported as a deformation.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents/complaints from this lot.The investigation was unable to determine a conclusive cause for the reported and noted difficulties. In this case, analysis on the returned device was unable to confirm a tip deformation; however, polymer coating damage near the tip was noted. The polymer damage may have been inadvertently reported as a deformation.Factors that may contribute to polymer coating damage include, but are not limited to, inadvertent mishandling during unpackaging, excessive force applied to device during unpackaging, and/or interaction with the dispenser hoop or an accessory device. In this case, analysis on the returned device noted that on the distal end of the polymer separation, the polymer was folded and stretched over itself. This type of damage is consistent with the guide wire interacting with another device.Additionally, scratches on the polymer were also noted, further indicating an interaction with another device.Based on the information reported by the account, it cannot be determined what caused the damages noted on the returned device. There is no indication of a product quality issue with respect to manufacture, design, or labeling.
