Event or Problem Description
|
|
Device report from synthes reports an event in (b)(6) as follows: it was reported that on (b)(6) 2022, the patient underwent a revision case due to extension of fusion, where the patient required l5-s1 fusion 10 years after the initial fusion of l4-l5 by the same surgeon. The surgeon¿s plan was to remove the 4 locking caps from the primary case, remove the rod, insert two screws in s1 and then add a new longer rod, and put in 6 new locking caps. After the first locking screw removal, it was noted that the driver shaft (used with torque handle) was burred. This was replaced with a new shaft and the case was completed as planned without any delay. No fragments were noted in the patient, and there was no patient consequence. This report involves one straight tip t25 driver standard. This is report 1 of 1 for (b)(4).
