According to the reporter, the child was on a vent part-time at home, the vent alarms were not constantly going off, therefore the balloon defect was not caught at home.Once the next trach was placed and the inflation of the balloon mechanism worked this was when the child's mother realized the trach cuff was broken. The mother went back and checked the cuff and it did not inflate. The ventilator alarms started alarming more often overnight then the cuff completely failed and required an early trach tube changed.
