Event or Problem Description
Medtronic received information regarding a shunt used during a shunt placement procedure. It was reported that the surgeon put in a valve on (b)(6) 2022.It was noted that the surgeon was "pretty sure" it was a regular size. The patient experienced no issues. The patient had an mri for a tumor follow up on (b)(6) 2022.The shunt was reprogrammed per normal after the mri to his setting of 2.5.There were no issues with the reprogramming. The patient had a new mri on (b)(6) 2022.The shunt was reprogrammed per normal to his setting of 2.5 with no issues.Once the surgeon had a chance to review the mri, she noted that the patient was "severely" over draining. There was a "massive" change on the scan compared to the one from (b)(6) 2022.The surgeon suspected that the explanation for this was that when it was reprogrammed in (b)(6), there was an issue with it.She sent him for immediate x-rays, same day as the new mri, and after it was reprogrammed a to 2.5 and the x-rays showed it was at 1.0.The patient came back to the office "today." the surgeon checked the shunt with the digital programmer and the "old school" one, both showed 2.5.Another healthcare professional put the magnet on it in the 1.0 spot and tried to ¿reprogram¿ ¿to 2.5.The programmer read 2.5.They sent him for x-rays, and the valve was still set at 1.0.There was no noted impact to the patient.
