The subject device has been returned to an olympus repair center for evaluation and inspection, and the customer's reported problem has been confirmed; a defective printed circuit board was found.This investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
It was reported by the customer that, during a laparoscopy procedure, the device sounded an alarm after 90 minutes of continuous operation.The operator of the device attempted to restart it to clear the error, with no success.The error could not ultimately be cleared, and as a result the device could no longer be used.It was exchanged during the procedure, which lengthened the procedure time by an unspecified amount of time.The extension of the procedure did not result in any adverse patient effects, and the procedure was completed successfully with the different device.There were no reports of patient or user harm associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely that the failure of the printed circuit board caused the front panel to turn off, an alarm to be generated, and an e03 to be displayed.The cause of the faulty circuit board could not be identified.Olympus will continue to monitor field performance for this device.
