Additional Manufacturer Narrative
|
|
Visual analysis was performed on the returned device. The reported tear/hole in the inner member within the balloon was confirmed.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar complaints reported from this lot.The investigation was unable to determine a definitive cause for the reported difficulties. However, based on the reported information and evaluation of the returned product, it may be possible that while advancing the ht command from the proximal end of the armada 14, the balloon may have been bent in the anatomy causing the distal end of the wire to puncture through the inner member and prolapse within the balloon. There were no leaks noted during preparation, suggesting that the damage to the inner member was not pre-existing and likely occurred during use. There is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.
