Event or Problem Description
During a supraventricular tachycardia procedure, the physician had difficulty trying to access the cs with the catheter resulting in a delay of the procedure.The catheter was noticed to be deformed (s shape) when the catheter was taken out of the body for checking.The catheter was replaced with another ibi-81104 and had the same issue.The catheters were then replaced with a livewire catheter and the issue was resolved.The case proceeded as per normal without any patient consequences.
Additional Manufacturer Narrative
One decapolar, inquiry steerable diagnostic catheter was received for evaluation.The catheter met specifications for electrical testing and deflected when actuating the steering mechanism in the correct shape according to specifications.An ¿s¿ shaped curve was not observed during testing.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed.The cause of the reported event of a deflection issue remains unknown as it could not be confirmed.
