Event Description
It was reported that the patient presented to the emergency department experiencing syncope/near syncope.It was observed that there was a possible shock for atrial fibrillation (af) with rapid ventricular response that the device classified as ventricular fibrillation (vf). the cardiac resynchronization therapy defibrillator (crt-d) remains in use.No further patient complications have been reported as a result of this event.

Event Description
It was further reported that the patient was hospitalized.It was noted the syncope was related to the ventricular arrhythmia and the shocks were appropriate.There was not intervention performed.
