The device was returned to olympus for evaluation and the customer's allegation was not confirmed.Additional information has been requested regarding this event.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or when additional information becomes available.
 
Event Description
It was reported to olympus by a customer that uhi-4 high flow insufflation had no air supply.The issue was observed at the end of surgery -total laparoscopic hysterectomy (tlh).There was no reported patient harm or impact due to this event.

This report is being supplemented to provide additional information based on the approved final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, and since no device malfunction was confirmed during evaluation, the definitive root cause of the air supply issue could not be determined.Olympus will continue to monitor field performance for this device.

Event Description
Additional information was received by the customer which confirmed the equipment was not inspected prior to use.The issue was observed at the end of a total laparoscopic hysterectomy (tlh) therapeutic procedure.No additional treatment or surgery was performed as a result of the event.The procedure was completed using the subject device without a delay.There was no reported patient harm or impact due to this event.
