Additional Manufacturer Narrative
Product complaint # (b)(4).Complaint conclusion: as reported by the field, the packaging of a cerebase 95cm catheter (gs9095sd, 30947381) was crushed during shipment, compromising the integrity of the sterile packaging, catheter, and accessory devices.Additional information was received clarifying that the device was never used and was received by the lab in the condition described above.A non-sterile cerebase 95cm catheter was received contained in the original outer box.Upon receiving the device, a visual inspection was performed, and it was noted that the box is compressed and already opened.Visual inspection of the inner pouch revealed that the pouch displayed steady transference from the material to the plastic seal, the sealing transfer line is continuous and adequate, indicating the pouch was fully sealed.Complaint history for product code gs9095sd (gs 90, 95 cm, straight, distal) lot 30947381, found no similar events reported. The device history records (dhr) for lot number 30947381 were reviewed and no evidence of deficiencies during the manufacturing process was found. No pieces were scrapped, and no non-conformances were generated during the manufacturing process.Based on the information reviewed and the evidence obtained from the returned device and inner pouch investigation, the customer complaint could not be confirmed, and there is no indication that the issue reported in the complaint is a result of a manufacturing defect. The compressed condition of the outer box found may be the result of the storage/handling of the device, this cannot be determined and is not considered a contributing factor that compromises the sterility of the device. It should be noted that product failure is multifactorial. The instructions for use (ifu) do contain the following precaution: do not use if the package or seal appears damaged, contents appear damaged, or the expiry date has passed. As part of the cerenovus quality process, all devices are manufactured, inspected, and released to approved specifications. As part of the post market surveillance program, information from this complaint is trended to identify statistical signals for consideration of further correction action.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.A supplemental report will be submitted if new facts arise which materially alter information submitted in a previous mdr report.
