Manufacturer's investigation conclusion: the reported event of the centrimag motor stopping during operation and not restarting was confirmed as the issue was reproduced during testing.The returned centrimag motor (serial number (b)(6) was connected to a known working test centrimag console upon arrival at the european distribution center; however, the motor was not recognized by the console and would not start.During testing when the motor and cable were moved, atypical motor and system fault alarms activated isolating the issue to the motor cable.Due to not being recognized by the console, the motor was unable to be functionally tested any further.The motor was advised to be scrapped.Per the customer¿s request, the motor was returned to the customer site labeled as ¿not for human use.¿ although not conclusively determined the reported event and reproduced issues during testing of the returned motor are consistent with wire fatigue within the centrimag motor cable.The root cause for the wire fatigue was not conclusively determined as the customer requested the motor be returned but is consistent with the repetitive flexing of the cable over time.Additional findings of kink and superficial jacket damage were noted near the motor-side bend relief.Review of the device history record for the centrimag motor, serial number (b)(6), showed the device was manufactured in accordance with manufacturing and quality assurance specifications.The 2nd generation centrimag system operating manual section 3 ¿warnings and precautions" warns "one additional 2nd generation centrimag primary console, motor and flow probe are required as backup system in the immediate vicinity of each patient whenever the centrimag or pedivas blood pump is used.The backup console must be connected to the backup motor and to the backup flow probe, have a battery charge sufficient for at least one hour of operation, be connected to ac power (except during transport) and be immediately available should the main console, motor or flow probe experience a malfunction." the centrimag motor instructions for use (ifu) instructs the user to inspect the centrimag motor, cable, console connector, and locking mechanism for any damage prior to use.If any component is damaged, do not use the centrimag motor.This document states that if the unit fails to operate according to the motor specifications or a console diagnostic error indicates a centrimag motor malfunction, it should be returned.Additionally, this document instructs the user to always have a spare centrimag motor and back-up equipment available.No further information was provided.The manufacturer is closing the file on this event.
