Due to the automated manufacturing execution system (mes) system there are controls in the manufacturing process to ensure the product met specifications upon release.During visual inspection the catheter was found to be broken off the hub, the catheter shaft was found to be kinked/bent in multiply areas, the tip and the hub were found to be intact.During functional inspection catheter shaft friction functional test was not performed due to visual defect noted. An attempt was taken to remove the sdw (stent delivery wire) from the catheter; however, it was not possible. The catheter was cut to remove the sdw (stent delivery wire). An attempt was taken to advance the patency mandrel to measure the id.It was difficult to flush the device, significant resistance was noted. The mandrel stuck 49 cm from proximal end and 9 cm from the distal end due to dry blood the reported event is covered in the device directions for use (dfu). As well, the risk of the reported event is documented in the risk documentation and there are current controls to mitigate the risk of the as reported event. The reported complaint was confirmed based on analysis. The device failed to meet specification when received for complaint investigation based on the analyzed anomalies noted to the device.Additional information provided by the customer indicated that there was no damage noted to the packaging prior to opening and the device was confirmed to be in good condition during preparation/prior to use on the patient. Also, additional information received indicated that patient anatomy was severely tortuous which would have contributed to the event. It was seen that the microcatheter was kinked which would have caused difficulty advancing the stent during the procedure. The microcatheter shaft most likely broke trying the advance/retract the stent during the procedure. The catheter was most likely damaged during the procedure due to some procedural/anatomical factors encountered during use leading to the reported and analyzed defects.Based on the investigation results and available information, an assignable cause of procedural factors will be assigned to the as reported.
