Event or Problem Description
Medtronic received a report that premature detachment occurred.  the patient was undergoing surgery for treatment of a saccular, unruptured left middle cerebral artery aneurysm with a max diameter of 5mm and a 2.7mm neck diameter. It was noted the patient's blood flow was normal and vessel tortuosity was normal. No patient symptoms or further complications were reported as a result of this event. It was reported that angiography showed that the tumor was located in the brain, using guide wire (model m00326010, lot number: 0000053549) and catheter (model: mc172150sx , lot number: 20062553p).When the apb-3-6-3d-es was opened, the coil detached itself in the microcatheter, which lead to the coil unable to be pushed and then it was withdrawn. The same model product was replaced, and the operation was successfully completed.  it was noted that coil separation/break/premature detachment occurred. The implant coil was removed from the microcatheter from the patient. There was no surgical or medicinal intervention required. The pushwire was not bent or broken. There was no friction or difficulty during delivery. It was unknown if the physician repositioned the coil. The physician did not attempt to detach the coil and did not rotate the delivery pusher during the procedure.Continuous flush was not administered.

Event or Problem Description
