Event or Problem Description

Additional information received via legal department -revision surgery operative report on (b)(6) 2021.There was a very large effusion of the knee with cloudy yellow fluid, this was collected and sent for fluid culture. The knee was inspected. There was near total catastrophic failure of the polyethylene involving the post and medial compartment with moderate polyethylene loss in the lateral compartment. There was extensive synovitis. There was osteolysis involving the proximal tibia and distal femur.A complete synovectomy was performed for purpose of removal of all polyethylene debris as well as exposure. The femoral component was able to be removed as it was debonded from the cement mantle. The cement was removed and revealed extensive osteolysis involving the medical and lateral femoral condyles and into the distal femoral metaphysis. The tibial component was removed in a similar fashion, and this also had large osteolytic lesion involving the metaphysis. The patella component was inspected and found to be fixed to the patella.A sterile bandage was applied, and the patient was transferred to the recovery room having tolerated the procedure well. There is no other patient demographic or medical history available. There is no device return. There are no photos or other images of the device provided. No additional information is available.

H3: investigation results - the revision reported may have been the result of polyethylene wear, osteolysis, and instability as stated in the operative notes. The extent and root cause of the reported polyethylene wear and osteolysis cannot be determined as the devices were not available for evaluation, and pre-revision radiographs and images of the explanted devices were not provided.Additionally, a contributing factor to the severe wear reported may have been the result of being packaged in a non-conforming vacuum bag for more than five years.
