Additional Manufacturer Narrative
Product complaint # (b)(4) section b5: additional information received confirmed that the resistance was first felt inside the mc.But we could not get the detail information. The event did not result in loss of cerebral target position. No damage was reported to the mc during manipulation of the coil or noticed to be damaged upon removal from the patient.Nothing was noted to be obstructing the microcatheter. The introducer was flushed until liquid was visible at the distal end of the slit in the clear tube. There were no procedural delays due to the event. No excessive force was applied to the device.Section e1.Initial reporter phone: (b)(6).Complaint conclusion: as reported by the field, during a stent assist coil embolization for a 20mm aneurysm in the middle cerebral artery (mca), an enteprise ii intracranial stent was implanted.Hydro coil(s) were used for framing from a sl10 microcatheter (mc).Several target xl coils were implanted and a micrusframe18 16mm x 47cm coil (mfr181647, (b)(6)) was used but became stuck on the microcatheter. This coil was replaced with another micrusframe18 14mm x 47cm coil (mfr181447, (b)(6) but became stuck during coiling and unraveled. The unraveled complaint coil was removed, and competitor¿s coils were used, and the procedure was completed. There was no negative impact on the patient.A continuous flush was done.Other concomitant devices are a microcatheter (exselsior sl10), a y-connector (terumo) and an intermediate catheter (navien).Additional information received confirmed that the resistance was first felt inside the mc.But we could not get the detail information. No excessive force was applied to the device. The device was discarded; therefore, no further investigation can be performed.A manufacturing record evaluation was performed for the finished device (b)(6) number, and no non-conformances related to the malfunction were identified.Impeded in microcatheter is a known potential product failure associated with the use of the device. The instructions for use (ifu) warns that if the microcoil system becomes immobile in the infusion microcatheter, apply a gentle push-pull motion to free it.If unsuccessful, remove both microcatheter and microcoil system together as a unit and replace with new devices. With the information available and without the product available for analysis, the reported customer complaints could not be confirmed.Based on the manufacturing record evaluation, there is no indication that the events are related to the device manufacturing process. The exact cause of the event could not be determined; however, there are circumstances of the procedure that may have contributed to the reported failure. As part of the post market surveillance program, information from this complaint is trended to identify statistical signals for consideration of further correction action.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.A supplemental report will be submitted if new facts arise which materially alter information submitted in a previous mdr report.
