Event Description
|
Medtronic received information that during implant of this transcatheter bioprosthetic valve, upon loading the in the compression loading system (cls), there was difficulties loading the valve.A new cls was used to load the valve.A dislodge of the valve occurred, causing ventricular tachycardia and low pressures.Epinephrine was used.A new transcatheter valve was used and implanted successfully.No additional adverse patient effects were reported.

Additional information was received which reported that there was nothing of note in terms of patient anatomy that contributed to the dislodgement.The reason for the dislodgement was not known, however the valve was deployed high.The deployment starting point was the bottom of the pigtail catheter.The valve dislodged aortic, occluding the coronary arteries.Following the dislodgement, the depth was -2 mm on the non-coronary cusp (ncc) and negative 3 on the left coronary cusp (lcc).The dislodgement of the valve resulted in the vtach.Of note, a non-medtronic safari wire was used.
