Event Description
It was reported that the patient's pump had flipped in the pocket and the patient had gone through some baclofen withdrawal.The dia gnostics/troubleshooting performed was the rep was unable to read the pump prior to going into the operating room.The actions/interventions that were taken to resolve the event was on 2023-02-15, the hcp opened the pocket and found approximately 10ml of pus and explanted the pump.The hcp opened the spinal site and cut the catheter to see if there was csf flow.It was reported there was csf f low and no sign of infection was seen in the spinal incision.It was reportedsome of the csf fluid was sent for cultures and the pump pocket pus was also sent for cultures.The hcp decided to implant the new pump at the posterior flank position on the right side due to infection in the pump pocket. all incisions were washed and closed and the patient's pump was reprogrammed by the physician assistant. the issue was resolved at the time of this report and the patient¿s status was ¿alive- no injury¿. the patient¿s weight and medical history were asked but unknown.

Additional information received from the company representative (rep) indicated the correction of gablofen dosing to be 200 mcg/day instead of 200 mg/day.
