Medtronic received information that during the implant of this transcatheter bioprosthetic valve, the valve was released at a depth of 2 millimeters (mm) on the non-coronary cusp (ncc) and 1 mm on the left coronary cusp (lcc).It was reported that upon release, the valve did not move.During removal of the delivery catheter system (dcs), the nose cone caught on the outflow crown of the valve.Subsequently, the valve dislodged.A second valve was implanted in the patient.No additional adverse patient effects were reported.
