Event or Problem Description
Information was received from healthcare provider (hcp) via a manufacturer representative regarding a generator and a handpiece. It was reported that while the hospital was performing a bi-ventricular ppm upgrade. It was a complex case as they were putting the lead in the right subclavian vein, and then tunneling across to the left side where the existing device was already inside to attach and screw in the lead (the left subclavian vein was too narrow to introduce another lead). The patient was pacemaker dependent. they had performed the first part of the operation and had moved to the left side to access the existing device using diathermy (plasmablade). The doctor was using the cut function (6) and it caused interference with the signal from the pacemaker resulting in loss of capture and therefore asystole.
