|
Information was received from multiple sources (healthcare provider, device manufacturer representative) regarding a patient who was receiving baclofen (unknown) (2000 mcg/ml at unk mcg/day) via an implantable pump for unknown indications for use.It was reported that the patient's pump was in shelf state.The caller stated that today at 12:06 pm a critical alarm that the pump had defaulted to minimum rate.The caller was unsure of why it happened and stated that they would call the doctor right away and have them try to reprogram and input the correct information.

Additional information was received from the company representative.The pump log error of shelf state was confirmed on the date that it was identified.The pump was programmed and read after the end of the session and pump resumed to normal operation.There is no further information currently.The information was confirmed with the healthcare provider.
