It was reported that shaft break occurred. The target lesion was located in the non-tortuous and moderately calcified ostial left anterior descending artery.A 4.00 x 16mm synergy xd drug-eluting stent was advanced to treat the lesion. The stent was deployed and inflated at 16 atmospheres with no issues. However, upon deflation of the balloon delivery system, the distal shaft of the stent at roughly 10 inches from the blue/white hub broke. Both the broken piece near the hub and the stent delivery system were manually pulled and were able to be easily removed from patient's body. The procedure was completed. There were no patient complications reported.
