A visual and chemical analysis was performed on the returned device. The reported stretched coils and contamination were confirmed. The reported break could not be confirmed.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history of the reported lot did not indicate a lot specific quality issue. The investigation was unable to determine a conclusive cause for the reported difficulties.Factors that may contribute to stretched coils upon unpackaging include, but are not limited to, damage incurred during manufacturing, damage incurred during shipping, or inadvertent mishandling during unpackaging.Factors that may contribute to contamination include, but are not limited to, contamination incurred during manufacturing, contamination incurred during shipping, or inadvertent contamination during unpackaging. In this case, a chemical analysis was performed on the returned device to investigate the reported corrosion on the guide wire.Chemical analysis confirmed that no corrosion was present on the guide wire.A conclusive source of the reported contamination cannot be determined due to handling of the guide wire after unpackaging, exposure to elements during transit back to abbott, and due to the decontamination process the device underwent upon return.Additionally, a visual and dimensional inspection was performed on unused/sterile units from the same part and lot number. The inspection found no abnormalities, contamination, or coil damage.Furthermore, a cause for the reported stretched coils in unable to be determined. There is no indication of a product quality issue with respect to manufacture, design, or labeling.
