This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, although the device was not returned for an evaluation, it was determined that an abnormality of the device was not the cause of the event. As reported, the nurse may have assumed that the device (model # fb-230u) was not sharp enough and may have torn the patient's tissue, causing the bleed. However, the root cause of the reported event could not be identified. The event can be detected/prevented by following the instructions for use (ifu) which state: ¿do not force the distal end of the insertion portion against body cavity tissue.Doing so could cause patient injuries, such as perforation, bleeding, or mucous membrane damage.¿ ¿biopsy may cause bleeding. If you take a large sample or press the instrument¿s distal end against tissue excessively, the risk of bleeding will increase.Perform biopsy on minimum necessary spots only.¿ this supplemental report includes a correction to e3 and g2 to provide information that was inadvertently not included in the initial medwatch.Olympus will continue to monitor field performance for this device.
