Additional information was received on 10 mar 2023: it was not the tip of the sheath that broke, however, it was the end of the 0.021 nitinol guidewire that comes in the kit that was retained in the patient, possibly from shearing and unravelling off the entry needle. The incident occurred while trying to pull the wire back through the entry needle. There was no tortuosity seen on imaging.Subclavian vein stick. At the time of incident, there was nothing else inserted into the sheath.

This report is being submitted as follow-up no.1 to provide additional information in section b5 and to provide the completed investigation results. The actual device was not returned; therefore, an evaluation of the actual device was unable to be conducted. The complaint can be confirmed for guidewire separation because the x-ray provided by the account shows the sheered guidewire tip in the chest cavity. The exact root cause could not be determined. The likely root cause is the guidewire was withdrawn through the access needle causing the guidewire to sheer and separate.Review of device history record (dhr) showed there were no issues noted during manufacture of the product that could have contributed to this complaint condition.Currently no action is recommended since this risk evaluation is within the predetermined limits in the design failure mode & effects analysis (dfmea).
