Additional Manufacturer Narrative
Blank fields on this form indicate the information is unknown, unavailable, or unchanged.Investigation ¿ evaluation (b)(6) hospital (republic of korea) informed cook on 07mar2023 of an event that occurred on (b)(6) 2023 involving an tscf-35-80-3 (safe-t-j fixed core wire guide) from lot 15076411.It was reported that when the user removed the wire, they found out that the coiling on the wire was peeling off. There were no adverse effects reported to the patient due to this occurrence.Reviews of the complaint history, device history record (dhr), instructions for use (ifu), and quality control procedures of the complaint device were conducted during the investigation. The complaint device was not returned; therefore, no physical examinations could be performed. However, an image of the reported device was received from the customer showing the wire guide to be unraveled.Additionally, a document-based investigation evaluation was performed.A review of the device master record (dmr) concluded that sufficient inspection activities are in place to identify this failure mode prior to distribution.A review of the dhrs for the reported complaint device lot revealed no recorded non-conformances relevant to the failure mode.A database search revealed one other complaint from the complaint lot from the same user facility and for the same failure mode.An expanded dhr was performed.Additional lots of the same rpn which were packaged within two weeks ((b)(6) 2022 ¿ (b)(6) 2022) of the complaint lot were reviewed.Twenty-four additional lots were found. No other additional complaints or related nonconformances were noted from these lots.Based on the available information, cook has concluded that the device was manufactured to specification and that there is no evidence suggesting nonconforming product exists either in house or in field.Cook also reviewed product labeling. The ifu [t_fcwg_rev2] packaged with the device contains the following in relation to the reported failure mode: "warnings: avoid manipulating or withdrawing the wire guide back through a metal needle or cannula.A sharp edge may scrape or shear material from the wire guide." based on the available information, imaging provided, and the results of the investigation, cook has concluded that failure to follow instructions led to this failure. The customer stated the unraveling occurred while within the needle. The unraveling likely occurred due to the user drawing back the wire during manipulation. The appropriate personnel have been notified.Cook will continue to monitor for similar complaints.Per the risk assessment no further action is required. This report is required by the fda under 21 cfr part 803 and is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement contained herein is intended to be an admission that any cook device is defective or malfunctioned or that a death or serious injury occurred; nor is it admission that any cook device caused, contributed to, or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
