Additional Manufacturer Narrative
|
|
A visual inspection was performed on the returned device. The reported stretched coils were confirmed.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history of the reported lot did not indicate a lot specific quality issue. The investigation determined the reported stretched coils appear to be related to the operational context of the procedure. There was no damage or anomalies noted to the guide wire during the inspection prior to use or during preparation for use which suggests a product quality issue did not contribute to the reported difficulties. In this case, it is likely that the guide wire interacted with an accessory device or the patient¿s anatomy during use, resulting in the reported stretched coils and noted bent shaping ribbon. There is no indication of a product quality issue with respect to manufacture, design, or labeling; therefore, no product-related corrective action will be implemented in this case.H6 - medical device problem code 1562 was removed and replaced with code 1601.
