This report has been identified as b.Braun medical internal report number (b)(4).One (1) used syringe without packaging was returned for evaluation.Visual evaluation of the syringe did confirm the plunger to be stuck in the barrel that was filled with an unknown residual fluid. The plunger could not be moved at all, and the glass appeared to be clouded, also with the residual fluid.Visual evaluation of the returned sample confirmed the reported defect.Although the defect was confirmed, the exact root cause of the crack was not able to be determined at this time.Review of the discrepancy management system (dsms) database was performed for the reported lot number and no abnormalities or non-conformances were noted during the in process or final product inspection.Additionally, a historical review of the customer complaint database dating back one year revealed no other complaints of this nature against this finished good item or lot number. Therefore, this is considered to be an isolated incident and no formal corrective action is warranted at this time.We will maintain this report for further references and continue to monitor other reports for similar occurrences. If any additional pertinent information becomes available, a follow up will be submitted.
