H6: medical device problem code.Section h3, h6: the devices were not returned for evaluation. However, the photographs were reviewed, and revealed that the insert appears to have scratches and wear near the base of the post. The clinical/medical investigation concluded that, the x-rays confirm the reported osteolytic changes between the femur and the femoral implant.Although, they do not aid in determining the clinical root cause of the reported insert damage, osteolysis, and loosening. It is possible that the loosening could have caused the femoral component to impinge on the insert post so cannot confirm that the poly damage came before the loosening.Based on the limited information provided, there were no clinical factors found which would have contributed to the reported event. The impact to the patient beyond the revision surgery cannot be determined. No further clinical/medical assessment is warranted at this time.Devices batch numbers were not provided, thus, an evaluation of the manufacturing records could not be performed.A review of complaint history for the previous 12 months did not reveal similar events for the listed devices.A review of the instructions for use documents for knee systems revealed that looseness of components has been identified in possible adverse effects section as a result from trauma, improper implant selection, improper implant positioning, improper fixation, and/or migration of the components.Muscle and fibrous tissue laxity can also contribute to these conditions.Besides, with all joint replacements, asymptomatic, localized, progressive bone resorption (osteolysis) may occur around the prosthetic components as a consequence of foreign-body reaction to particulate wear debris.A review of the risk management files revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to these products and event. At this time, we have no reason to suspect that the products failed to meet any specifications at the time of manufacture.Factors that could contribute to the reported event include abnormal motion over time, bone degeneration, size selected, inadequate integration between the cement and bone/implant, osteolysis, traumatic injury, patient medical history, post-operative patient health, adverse reaction and/or bone quality.Based on this investigation, the need for corrective action is not indicated.Should the devices or additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.H6: health effect - clinical code.
