Additional Manufacturer Narrative

Manufacturer¿s ref no: (b)(4).Procode is krd/hcg.The pre-shipment photos of the complaint devices included in the file were reviewed.Based on the photos, the hypotube (delivery tube) of the complaint device was observed kinked. There was no embolic coil observed to be attached to the rh (resistance heating) coil. The appearance of the outer sheath of the rh coil does not suggest that the embolic coil became detached as a result of the initiation of the detachment cycle.Rather, it appears that a mechanical detachment occurred. The embolic coil component was observed kinked inside the introducer. The coil suture was observed snapped on the proximal end of the embolic coil, which confirms that the embolic coil was mechanically detached.Based on complaint information, the device is not available to be returned for analysis.Product analysis cannot be conducted as the product was not returned for analysis. No determination of causes and possible contributing factors could be made.As such, the investigation will be closed.A review of manufacturing documentation associated with this lot: (30871979) presented no issues during the manufacturing or inspection processes related to the reported complaint. There were no nonconformances related to device manufacture or inspection. As part of the cerenovus quality process, all devices are manufactured, inspected, and released to approved specifications. The reported issue documented in the complaint that the embolic coil was impeded in the concomitant microcatheter was confirmed based on the damages observed in the photos provided in the complaint. The damages observed appeared to be the result of the coil getting stuck in the microcatheter. This investigation was performed based only on the photo provided. If the product is received after this investigation, an assessment will be performed as per the conditions of the device returned. With the information available and without the complaint product available to be returned for analysis, the reported issue in the complaint cannot be confirmed.Based on the manufacturing documentation review, there is no indication that the event is related to the device manufacturing process.Assignment of root cause for the event remains speculative and inconclusive, based on the limited information provided and without the return of the complaint sample; however, it is possible that clinical and procedural factors, including device manipulation / interaction may have contributed to the reported failure. As part of the post market surveillance program, information from this complaint is trended for statistical signals and corrective / preventive action may be triggered at a later time.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time. This is one of 3 products involved with the reported complaint. The associated manufacturer report numbers are: 3008114965-2023-00211, 3008114965-2023-00212.This report is being submitted pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been investigated or verified prior to the required reporting date. This report does not reflect a conclusion by cerenovus, or its employees that the report constitutes an admission that the product, cerenovus, or its employees caused or contributed to the potential event described in this report. If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate. The manufacturer will submit a supplemental report if new facts arise which materially alter information submitted in a previous mdr report.Additional information will be submitted within 30 days of receipt.

Manufacturer¿s ref.No: (b)(4).The purpose of this mdr submission is to report the investigation finding of the returned device completed on 19-apr-2023, and the additional information received on 20-apr-2023.On 20-apr-2023, additional information was received. The information indicated that the resistance was first felt inside the microcatheter, however, no further details can be provided. It was not necessary to remove or replace the microcatheter. No damages were reported on the devices.Nothing was noted to be obstructing the microcatheter.Excessive force had not been applied to the device. The complaint device was returned for evaluation and analysis. The investigation finding is documented below.Investigation summary: a non-sterile 2mm x 8cm galaxy g3 xsft helical was received contained in the decontamination pouch.Visual inspection was performed. It was noted that the introducer was separated from the rest of the unit. The core wire was found to have torsion damage just next to the distal marker band. The device was inspected under the microscope and the embolic coil was found halfway inside the introducer. The coil was observed to be kinked and detached from the resistance heating (rh) coil, which was observed not softened, an indication that the detachment process had not been initiated. The conditions observed are consistent with the damages noted in the pre-shipment photos. It was also noted that the introducer tip was found saturated with residues of dried blood. The issue reported that the coil became impeded in the microcatheter and could not be delivered, could not be evaluated through functional testing due to all the damages observed on the coil and core wire components.According to the risk documentation, coil damage and device damage (i.E., core wire) are potential issues that can occur during microcoil placement due to the rotating hemostasis valve (rhv) being fastened too tightly and the introducer tip and microcatheter hub being misaligned. Also, friction and difficulty to advance are potential issues that can occur during microcoil placement due to continuous saline flush not being established, which can result in coil kinking. However, other factors not described in the event description may have contributed to the issue encountered during the procedure.Based on these findings, the customer complaint regarding a coil impeding in the microcatheter was confirmed. With the information available there is no indication that the issue reported in the complaint results from a defect inherently related to the device. The coil detachment was not originally documented in the complaint but based on the position of the coil within the introducer, it should have occurred when the coil was already removed from the microcatheter and reloaded into the introducer. There is no evidence to suggest that this finding is related to the reported failure.A review of manufacturing documentation associated with this lot (30871979) presented no issues during the manufacturing or inspection processes related to the reported complaint. There were no nonconformances related to device manufacture or inspection. As part of the cerenovus quality process, all devices are manufactured, inspected, and released to approved specifications. It should be noted that multiple factors could cause product failure. The instructions for use (ifu) does contain the following recommendations: ¿ do not fasten the rhv valve too tightly around the introducer sheath since excessive pressure may cause damage to the introducer sheath and/or the microcoil as it is advanced into the infusion microcatheter.Additionally, if the introducer tip and microcatheter hub are misaligned, damage may occur to the microcoil as it passes through this transition.¿ if unusual friction is noticed during advancement or retraction of the microcoil system through the introducer, open the rhv main valve, and partially withdraw the distal end of the introducer to expose its tip within the rhv.Tighten the rhv main valve and flush the y-connector of the rhv with sterile saline and verify that fluid exits the slit in the clear portion of the introducer.¿ if unusual friction is still noticed during advancement or retraction of the microcoil system, verify flush lines are open and properly pressurized. Then slowly withdraw the entire microcoil system and examine for damage.Replace it with a new microcoil system. If friction still exists, withdraw, and examine the delivery catheter system.Based on the manufacturing documentation review, there is no indication that the event is related to the device manufacturing process. As part of the post market surveillance program, information from this complaint is trended for statistical signals and corrective / preventive action may be triggered at a later time.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.Section e.1: initial reporter phone: (b)(6).This is one of 3 products involved with the reported complaint. The associated manufacturer report numbers are: 3008114965-2023-00211, 3008114965-2023-00212.The manufacturer will submit a supplemental report if new facts arise which materially alter information submitted in a previous mdr report.Additional information will be
