Additional Manufacturer Narrative
H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The complaint of missing depth markings on the catheter was confirmed and appears to be related to the manufacturing process.A single photograph of a picc catheter was returned for evaluation. The proximal end of the device was not visible in the photograph, so the configuration could not be verified, but the sample appeared consistent with the implicated 5fr s/l powerpicc catheter. The catheter shaft appeared to be missing depth markings. The printing disappeared at the 33cm depth marking and then reappeared again near the distal end of the catheter. The exact number of depth markings missing from the catheter could not be determined from the photograph but appeared to be approximately 19 centimeters worth or printing.A review of the catheter drawing confirmed that the catheter shaft should contain continuous depth markings from 0cm through the 54cm marking dot.As the missing depth markings were noted prior to placement of the device, the most probable cause is due to the manufacturing printing process.Bd is working closely with manufacturing to help prevent recurrence of the reported event.H3 other text : evaluation findings are in section h.11.
