Event or Problem Description
An abre self-expanding stent was used for treatment of a lesion in the iliac artery both common and external.A 9fr sheath was used. The lesion was not predilated it was reported the physician a placed a 16mm x 150mm abre stent in the patient¿s left common into the external iliac.A small part of the stent is in the vena cava. After placing the stent, ivus was used to measure the stent and it measured 18mm+.The physician believes the stent is an 18mm stent but was packaged in a 16mm box/packaging.The patient is planned to be brought back in 3 weeks to stent the other side due to the stent measuring 18mm+ and a portion of it is in the vena cava and infringing on the right side of the vena cava.No further patient injury reported for this event.

Additional Manufacturer Narrative
Additional information: the lesion being treated was the left common and external iliac vein narrowing confirmed with ivus.Image review: it appears they are measuring outer wall of the strut/stent -it appears they undersized the native vessel wall.In this two dimensional image, the vessel wall is larger than the stent, and the refore the native vessel wall would be >18mm (so likely good that they didn't use a 16mm).Perhaps, their ivus is not calibrated? unable to comment on stent protruding into vena cava from these images, but would request venogram or ct.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report. In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿.These words are included in the fda 3500a form and are fixed items for selection created by the fda to categorize the type of event solely for the purpose of regulatory reporting.Medtronic objects to the use of these words and others like them because of the lack of definition and the connotations implied by these terms. This statement should be included with any information or report disclosed to the public under the freedom of information act.
