Event or Problem Description
It was reported that a 64-year-old caucasian female patient underwent a primary breast augmentation surgery with implantation of a 125cc mentor smooth round moderate profile saline breast implant prosthesis.Post-operatively, the patient experienced right breast soreness, and the following day observed the right breast had deflated.During a consultation with a medical professional, she was diagnosed with a deflated right breast implant. As a result, the patient underwent bilateral removal and replacement with 300cc mentor smooth round moderate plus profile saline breast implants on (b)(6) 2023.

Additional Manufacturer Narrative
On may 11, 2023, mentor was notified that the original device information was incorrectly reported by the healthcare professional. The affected right implant was clarified and the following information was updated. The catalog numbers for the two devices are the same.Lot: 5553001.Serial number: (b)(6).On may 19, 2023, mentor completed an evaluation on the returned device.Mentor conducted a visual inspection, leak testing, and microscopic examination of the device.Visual analysis of the returned sample revealed that the breast implant had a crease/fold on the anterior view extended to the posterior view.Leak testing was performed, according to mentor procedure, and it identified a tear within the crease/fold on the anterior view, measuring approximately 0.7 cm.Additionally, no other leak sites were detected. The evaluation determined that the cause of the rupture is consistent with normal wear.Deflation can occur at any time after implantation, but it is more likely to occur the longer the implant is implanted.Creating wrinkles or folds should be avoided during implantation or other procedures as it can result in deflation at a later time.Breast implants may also simply wear out over time. Most women undergoing augmentation or reconstruction with a mammary prosthesis will experience some pain postoperatively. While pain normally subsides in most women as they heal from surgery, it can become a chronic problem in other women.Chronic pain can be associated with a variety of factors.Surgeons should instruct their patients to inform them if there is significant pain or if pain persists.Pain is a known complication associated with these devices and is referenced in our current product insert data sheet.A manufacturing record evaluation (mre) was performed for lot 5553001, and no anomalies were found related to this complaint. In addition, the mre verifies that the device was manufactured in accordance with documented specification and procedures.Manufacturer¿s reference number: (b)(4).
