Event or Problem Description
The patient was initially implanted with an afx2 bifurcated stent graft and an afx vela infrarenal stent graft extension to treat an abdominal aortic aneurysm (aaa).Approximately (2) two years post initial procedure a type 1a endoleak was identified. As a result, the physician intends to perform a re-intervention.

Additional Manufacturer Narrative

The reported event/incident was investigated in alignment with endologix operating procedures and work instructions.Where possible, it is endologix practice to make at least three good-faith efforts to retrieve a reported adverse event/incident-related device as well as medical records and medical imaging. An evaluation of the manufacturing record was completed.A review of the part number/lot number combination needed for device identification shows the device to have been properly manufactured and released in accordance with the device master record. The review confirms there were no manufacturing or processing non-conformities identified that would contribute to the reported event/incident.The device was not returned to endologix for evaluation because it remains implanted.A clinical evaluation of the adverse event/incident was completed. An examination of medical records and/or medical imaging received by endologix shows the type 1a endoleak of the proximal extension with an additional endovascular procedure is confirmed. This is consistent with the reported adverse event/incident.The clinical evaluation also shows reasonable evidence to suggest an aneurysm enlargement of 9.6 mm, a type ii endoleak, and movement of the infrarenal proximal extension of 5.7mm also occurred. The non-device-related type ii endoleak is anatomy related. These findings were discovered during an examination of the 10- and 27-month post ct (computed tomography) scans and angiograms dated 12/23/2020 and 03/20/2023.The complaint is most likely user related. It was noted that there was suboptimal placement of the infrarenal proximal extension at index as indicated on the angiogram dated 12/23/2020; the device's initial placement as well as its postoperative movement likely contributed to the reported type 1a endoleak. In addition, there was significant proximal neck thrombus at the index procedure (cautionary product use condition). No procedure-related harms were identified. The final patient status was reported as discharged home on postoperative day one.No additional investigation of this reported event/incident is planned. However, should additional information relevant to the investigation outcome become available, a follow-up report will be submitted.Endologix will continue to monitor this and similar events/incidents.Device iteration is afx with duraply.Corrections: b2 outcomes attributed to ae b5 describe event or problem g3 awareness date h6 medical device problem codes; remove 4065 h6 investigation finding codes; remove 3233 h6 investigation conclusion codes; remove 11.
