During a left heart catheterization (lad) for coronary artery disease (cad) on a 73-year-old female patient, air bubbles were injected into the patient's right coronary artery (rca); column of air with no flow past the proximal segment.Patient experienced blood pressure of less than < 90 mmhg systolic, st-segment elevation on the ekg, hypoxia, and chest pain. The patient was administered heparin, neosynephrine / levophed, and placed on oxygen.Guidewire introduced to rca, pronto thrombectomy cath performed, and patient was given a dose of nitroglycerin and intracoronary adenosine. The repeat angiogram showed restoration of flow and the patient's symptoms subsided. The patient subsequently recovered.

The acist angiographic injection system, model cvi, system serial number (b)(6), was evaluated april 25, 2023.The injection system was functionally tested and met the pre-established specifications. There was no evidence of device malfunction related to the reported event. The instructions for use have been reviewed and no inadequacies were identified regarding warnings, contraindications, and the directions/conditions for use of the device.Per the acist cvi user's manual, the air column detect sensor is designed to aid the user in the detection of air columns in the injection line, but it is not designed to replace the vigilance and care required of the operator in visually inspecting for air and clearing air from the entire patient kit and angiographic catheter. The air column detect mechanism is to be used in conjunction with and to complement the user's other procedures for preventing air injections. The cine-angiograms were evaluated by the acist medical advisory board member and is as follows: i reviewed the report and angio films, and, as described, the angio shows a first image of an occluded right coronary artery (rca) that is consistent with a significant amount of air having been injected into the vessel prior to this angio image. This was the first image of the angio.Presumably, the air was not fully evacuated from the system during set up or air was not fully cleared from the tuohy/catheter.This air was likely injected during filling of catheter with contrast. It was cleared with aspiration.Subsequently, angiography of the left coronary was completed without incident. The customer reported that the facility's lead angiography technologist had received refresher training in set up, flushing and best practices. This report is closed.
