Other, other text: d3, g1, and g2 email is: (b)(4).Device evaluation: one device was returned for investigation in used condition without original packaging. The valve assembly was bent and front panel is slightly bowed between the gas supply indicator and manometer.Functional testing was done where the customer complaint was confirmed. The gas supply indicator did not change from red to white with gas supply. The gas supply tubing was stuck underneath the main board and the housing. This can only happen from shifting or removal of the front panel.Impact caused a bent patient valve set.Maintenance was done and device passed all functionality tests.Root cause most probably traced to impact and user interface issues. The product's history records were reviewed and there were no non-conformances nor service-related issues that would have resulted in the reported complaint.Installed mri battery, sintered filters, and o-rings for the preventative maintenance (pm).Re-shaped front panel.Adjusted peep/ cpap control and peak inspiratory control to tolerance.Performed pm, cleaned unit and affixed new service label.
