Based on the claim against the product by the customer noting an image mirroring issue, an investigation was completed to determine the cause of this reported event.Intuitive surgical, inc.(isi) did receive a da vinci product to perform failure analysis.The endoscope was analyzed and found to have endoscope adapter (aea) binding or friction issue.The profile tube was damaged, and contamination/corrosion was observed.The shaft was found to have mechanical damage.There was discoloration of housing and transmittance test failure.The complaint regarding image orientation issue was confirmed by failure analysis, which indicates that the device did contribute to the customer reported issue.This complaint is considered a reportable event due to the following conclusion: it was alleged that the endoscope image was mirrored.Poor camera control could result in unintuitive motion and subsequent tissue damage.At this time, the root cause of the failure is unknown.While there was no harm or injury to the patient, the reported failure mode could likely cause or contribute to an adverse event if it were to recur.

Event Description
|
It was reported that during a da vinci-assisted radical cystectomy with neobladder surgical procedure, the image from the 30-degree endoscope was mirrored.The procedure was completing as planned with no reported injury.Intuitive surgical inc.(isi) followed up with the initial reporter and obtained the following additional information: during the operation the instruments suddenly could no longer be controlled, and the camera image appeared double, and mirror inverted.After contacting the technician, the optics were replaced, after which all faults were rectified, and the operation was completed.
